Thank you for registering to subscribe to MAMMOCLUB for our mutual protection and support

YOUR ID

Your ID number is which you must quote on all correspondence.

SCREENING

You will report at and to our offices at Accra. Monday to Friday, between 9:30 a.m and 2:30p.m. for your screening. Thereafter you will present yourself for screening every 4 - 6months for which specific purpose notification will be given you. You must present your ID Card each time you visit us for a service.

BENEFITS

Your other benefits are as follows:

1. Free educational material and literature

2. Free treatment for every minor breast condition

3. Free for early detection services, surgery and 50% cost of chemotherapy

4. Emotional support for pre-treatment and post-treatment period

5. Breast reconstruction/plastic surgery for those who qualify

OBLIGATIONS

MAMMOCLUB provides emotional support for all members. You are therefore enjoined to fully participate in programmes which may be undertaken from time to time to demonstrate support in numbers and testimonies. You are also required to present yourself for screening upon notification.

ANNUAL SUBSCRIPTION

The services we provide are funded from your annual subscriptions. You are therefore enjoined to renew your subscription promptly at your anniversary of joining the club. Due notification will be mailed to you. Failure to do so within a month of notification will result in loss of membership and benefits for that year.

ONLY MEMBERS IN GOOD STANDING WILL BE ENTITLED TO BENEFITS
